  1. Personal Data:

    • Full name
    • Date of birth
    • Gender
    • Country of birth
    • Identity document (for Cuban citizens) or passport number
    • Passport issuing country

    • Email and phone number
  2. Immigration Information:

    The following section of the form is related to your trip and your immigration information. Here you must indicate:
    • Date of arrival in Cuba
    • Airline and flight number
    • Seat number on the plane
    • Entry Airport to Cuba
    • Country of origin
    • Reason for your trip (tourism, business, etc.)
  3. Health Information:

    • Province and municipality where you will stay in Cuba
    • Address of the accommodation place
    • Countries visited in the last 15 days

    • If your vaccination schedule is complete or incomplete
    • Name of the vaccine administered
  4. Customs Declaration:

    • If you are carrying unaccompanied luggage
    • If you are traveling with minors
    • Amount of foreign currency you are carrying (you must declare if you are carrying more than 5,000 USD)
    • Value of the miscellaneous items you bring (if they exceed 1,000 USD)
    • Declaration of equipment or items that are not considered miscellaneous, such as appliances
  5. Signature and QR Code:

    At the end of the form, you must sign an affidavit and enter a verification code. Once you have correctly completed the form, you will receive a PDF file with your data and a QR code that you must present to the Cuban customs authorities, whether printed or on your mobile device.
